Before attending your 1st counseling session, please download and then complete and bring the attached forms. This will allow you more time in your first session for counseling versus filling out intake paperwork.

If you are 15 or older, please fill out the Adult Disclosure Form.

If you are 14 or younger, please have your parent/guardian complete the Minor Disclosure Form.
